Recent advances in the treatment of Poor Ovarian Reserve
What Is Ovarian Rejuvenation?
Ovarian rejuvenation is a procedure that may create new eggs in the ovaries of women who are unable to conceive because of early menopause, advanced maternal age or low oocyte (egg) reserve, yet who wish to have their own biological child. These individuals are either unable or unwilling, for their own personal reasons, to use donor eggs or to adopt a child. New eggs do not develop in the ovaries under normal circumstances, and, according to the current scientific understanding of ovarian physiology, a woman is born with all the eggs that will be available for conception during her reproductive life. Furthermore, it is a scientifically undisputed fact that a woman’s supply of eggs diminishes both in number and genetic quality as she ages.
How Can Ovarian Rejuvenation Treatment Help Infertility
Once activated by growth factors after the body is alerted to an internal or external injury, stem cells can morph into any type of cell in the human body. That thick yellow substance that forms over a cut as it heels? That is a combination of clotting blood, white blood cells, platelets and growth factors working to defend against infection, stop bleeding, and gradually form new skin, blood vessels, nerves and connective tissue. It’s a natural process clinicians have harnessed for years to accelerate healing of injuries from sports-related damage to skin grafts applied to burn patients and cardiac-surgery incisions.
What Are Growth Factors?
When your body is injured, it dispatches substances known as “growth factors” to provide aid. Produced naturally by platelets and white blood cells, growth factors activate the body’s stem cells to repair tissue, and induce regrowth of connective tissue and blood vessels. Physicians have been utilizing growth factors to treat a range of soft or connective tissue injuries for years, since injecting platelet-rich plasma (PRP) from patients’ own bodies carries no risk of transmitting foreign diseases and infections. Additionally, there is no foreign blood products or man-made chemicals used in the process since it’s the patient’s own platelet rich plasma that is used.
How Is Ovarian Rejuvenation Procedure Performed?
The procedure can be performed anytime on women with or without periods.
Ovarian rejuvenation takes place following a two-step process. First, we will isolate your PRP by subjecting a tube of blood to centrifugation in order to separate white cells and platelets.
After a patient’s own blood is drawn, Ovarian Rejuvenation involves injecting a patient’s ovaries with her own platelet-rich plasma (PRP), infusing these oocyte-production centers with proteins rich in growth factors and stem-cell chemoattractants. It can be performed at any time on a woman who has stopped getting periods.
This stage takes under an hour. In the next and final stage, you will spend about 10 minutes under an intravenously administered anesthetic while we nonsurgically inject a small amount of PRP directly into your ovaries, a procedure guided by transvaginal ultrasound. Afterward, you will sleep around one hour before being discharged to return home accompanied by a family member or friend.
Due to the nature of the anaesthetic used, we ask that patients not drink or eat anything for at least eight hours before undergoing This is a simple, common procedure used during IVF under IV sedation by an anesthesiologist. There is minimal complications since this is similar to egg retrieval that has been common practice for years
Are There Any Risks?
The procedure carries roughly the same risks associated with In Vitro Fertilization (IVF) egg-retrieval: post-procedure pain, fever or internal bleeding that may require hospitalization in rare instances.
What Are The Benefits?
The greatest possible benefit? A chance to become pregnant using your own eggs. As opposed to the laparoscopic procedure utilized during initial research, we opt for the same procedures employed to retrieve eggs for In Vitro Fertilization (IVF) in the name of a safer, less-invasive process with much shorter patient recovery time. In an ideal outcome, you will have a chance to become pregnant using your own naturally developed mature eggs without risking introduction of disease, infection or synthetic chemicals to your body.
Who Is An Ideal Candidate For Ovarian Rejuvenation?
Any woman who is in good physical health and falls into one or more of the four patient categories that will be studied will be evaluated for admission to this study.
- Menopausal or perimenopausal women under the age of 50 years.
- Infertile women, over the age of 35 years, having low egg reserve and low AntiMullerian Hormone levels
- Women under the age of 35 years, who have low egg reserve and low AntiMullerian Hormone levels
- Women with premature ovarian failure (POF)
After the rejuvenation procedure, we would measure progress by evaluating and measure AMH (anti-Mullerian hormone), FSH, LH and Estradiol levels along with the ovarian function for 1-3 months. Once we see AMH levels rise and FSH, LH and Estradiol levels diminish, this would be a positive sign that the ovarian rejuvenation is working. Please keep in mind that the effect of growth factors could take 3-6 months of observation since this type of tissue transformation takes time, and every person’s situation may be different.
